Abstract:

After offering an overview of some of the main themes of Popper’s political thought, the paper argues that his account faces two PROBLEMS relating to institutions. The first is that while Popper stresses the ‘rational unity of mankind’, and the potential for any of us to furnish criticisms of public policy, it is not clear what institutional means currently exist for this to enable this to take place. Second, Popper has stressed the conjectural character of even our best theories. However, at any point, some theories will have fared better in the face of criticism than others, and they may give us important information about constraints on our actions. At the same time, as ordinary citizens we may not be in a good position to understand the theories in question, let alone appraise the state of the specialised discussion of them. There is, it is suggested, a case for thinking of ways to institutionally entrench such fallible theories, especially in the current setting in which social media play an important role

Abstract:

INTRODUCTION: OCCUPATIONAL THERAPY IS A HEALTH PROFESSIONAL WITH MISSION OF IMPROVEMENT OF HEALTH AND WELLBEING OF CLIENTS. THEORETICALLY, PERSON IN INTERACTION WITH ENVIRONMENT PERFORM OCCUPATIONS THAT LEAD TO OCCUPATION PERFORMANCE. OCCUPATION PERFORMANCE RESULT IN PERSON ADJUSTMENT WITH ENVIRONMENT, AND SATISFYING OF NEEDS. USING OCCUPATION BASED INTERVENTIONS; OCCUPATIONAL THERAPISTS TRY TO IMPROVE OCCUPATION PERFORMANCE IN DIFFERENT DISORDERS. SLEEP IS A BASIC OCCUPATION AND DISTURBED SLEEP CAN DISTURB OCCUPATION PERFORMANCE. THEREFORE, RESEARCH ON THE RELATIVELY NEW FIELD OF SLEEP IN OCCUPATIONAL THERAPY, AND HOW OCCUPATIONAL THERAPISTS CAN IMPLEMENT THEIR INTERVENTIONS FOR SLEEP PROBLEMS IS NECESSARY.MATERIAL AND METHOD: THE SEARCH WAS PERFORMED ON THE GOOGLE SCHOLAR, PUBMED, ISI DATA BASE USING SLEEP PROBLEMS, OCCUPATIONAL THERAPY, OCCUPATION PERFORMANCE KEY WORDS. RELEVANT PAPERS WERE EXTRACTED AND ANALYZED.RESULTS: A LARGE BODY OF EVIDENCE SHOWS THAT SLEEP PROBLEMS CAN EFFECT ON OCCUPATION PERFORMANCE, AND THERE ARE SEVERAL INTERVENTIONS TO TREATMENT OF SLEEP PROBLEMS, ESPECIALLY INSOMNIA. COGNITIVE BEHAVIOR THERAPY, PHYSICAL ACTIVITY, AND MULTI TREATMENT INTERVENTIONS ARE AMONG THE TREATMENT. EVIDENCE ALSO SHOW THAT OCCUPATIONAL THERAPIST CAN APPLY THE INTERVENTIONS FOR CHILDREN WITH AUTISM SPECTRUM DISORDERS, OLDER ADULT, MENTALLY ILL PATIENTS, AND SO FOR CLIENTS IN VOCATIONAL REHABILITATION PROCESS. HOWEVER, THERE IS NO DIRECT EVIDENCE TO SUPPORT EFFICACY OF OCCUPATIONAL THERAPY INTERVENTIONS IN SLEEP PROBLEMS.CONCLUSION: SLEEP PROBLEMS CAN AFFECT OCCUPATION PERFORMANCE, AND OCCUPATIONAL THERAPIST SHOULD IMPLEMENT THEIR INTERVENTIONS TO IMPROVE SLEEP QUALITY AND OCCUPATION PERFORMANCE.MORE EDUCATION AND RESEARCH IN THIS NEW FIELD OF OCCUPATIONAL THERAPY IS NECESSARY.

Abstract:

Background and Aim: SLEEP habits affect childhood health. Paying no attention to SLEEP PROBLEMS can have a negative effect on children's health and performance, both at home and school. SLEEP disorders have a relationship with illnesses, imperfect operations, decrease of life quality, and increase in living costs. The purpose of the present study was to pinpoint SLEEP pattern and SLEEP disorders in elementary school children in Birjand.Materials and Methods: In this descriptive and comparative study, 620 elementary students, who did not have any underlying diseases and had no history of taking psychogenic drugs, were selected through multi-stage randomized sampling were assessed. Assessment tool for SLEEP disorders in children (Bear's questionnaire) was filled out by parents. The obtained data was analysed by means of SPSS software (V: 15); and statistical tests t and X2 at the significant level P<0.05.Results: Out of 620 students, 302 (48.7%) were boys and 318 (51.3%) were girls. Mean age of them was 9.08±6.1 yrs. The most common SLEEP PROBLEMS in children were: resistance to going to bed (20.5 %) and chatting while SLEEPing (11.1%). SLEEP duration (in girls), resistance to SLEEP, enuresis, and noisy breathing (in boys) had a higher relative frequency. Conclusion: The most prevalent SLEEP PROBLEMS in children were resistance to going to bed and chatting while SLEEPing which require attention, following up, and treatment in order to prevent probable following complications.

Abstract:

INSOMNIA CAN BE PRIMARY OR SECONDARY TO OTHER PSYCHIATRIC OR MEDICAL CONDITIONS OR IT MAY BE COMORBID. IN SECONDARY CONDITIONS TREATMENT OF PRIMARY PROBLEM CAN IMPROVE SLEEP DISTURBANCE BUT IN SEVER INSOMNIA MAY NEED PHARMACOLOGICAL INTERVENTION AT LEAST FOR SHORT PERIOD OF TIME.PARENTS MUST BE INFORMED THAT THERE IS NOT FDA APPROVED DRUG FOR THIS PURPOSE AND MUST BE DISCUSS ABOUT COST BENEFIT OF THESE DRUGS. PHARMACOLOGICAL INTERVENTIONS MUST BE USED WHEN BEHAVIORAL INTERVENTIONS ARE INEFFECTIVE. IN SLEEP REGULATION NEUROTRANSMITTERS SUCH AS GABA, MELATONIN, HISTAMINE, NOREPINEPHRIN, ACETYLCHOLINE HAVE ROLE, AND THERE FOR DRUGS ARE USED FOR THIS PURPOSE AFFECT THEM. SEDATIVE EFFECT OF ANTIDEPRESSANTS LIKE TRAZODONE AND AMITRIPTYLINE IS RELATED TO ANTICHOLINERGIC MECHANISMS. THERE ARE VARIOUS FORMS ANTIHYSTAMINES AND ARE USED IN CHILDREN FOR INSOMNIA.ALSO ALPHA-ADRENERGIC AGONISTS SUCH AS CLONIDINE MAY BE USED FOR SEDATIVE EFFECT.
